1) If I have diabetes, epilepsy, hepatitis, hemophilia, HIV-AIDS or any other communicable disease, heart condition or I take medication that thins the blood, I have advised my artist. I am not pregnant or nursing. I am not under the influence of alcohol or drugs.

2) I acknowledge that there is a chance that I may be allergic to to inks or pigments used, and I have been informed of, or requested, a colour test in case of such a reaction. If a reaction does occur, the artist is not at fault.

3) I acknowledge that infection is always possible as a result of obtaining a tattoo, particularly in the event that I do not take proper care of my tattoo. I have received aftercare suggestions and agree to care for my tattoo to the best of my ability. I understand that my artist is always available to be contacted with any questions regarding said aftercare.

4) I understand that if I have any skin treatments, laser hair removal, plastic surgery or other skin altering procedures, it may result in adverse changes to my tattoo.

5) I acknowledge that tattoos are permanent (I mean, duh, but still).

6) I understand that my artist can be reached out to if I have any questions or concerns before, during, or after the tattoo procedure. They are available to answer questions regarding aftercare, upkeep, tattoo appointment preparations, etc.

7) I am at least 18 years of age and am obtaining a tattoo by my choice alone.
